
2025 Clinical Informatics Maturity Survey: Clinical Informatics Support, Governance and Digital Maturity
The 2025 Clinical Informatics Maturity Survey provides a contemporary dataset on workforce structures, governance arrangements, and reported digital maturity outcomes across NHS Trusts and Irish health organisations. Synthesised with longitudinal NHS England Digital Maturity Assessments (DMA), the What Good Looks Like (WGLL) framework, and HIMSS EMRAM™ benchmarking, the evidence suggests digital maturity is shaped by both technology adoption and sustained investment.
Key findings include:
- EMRAM™ score reporting was limited (n=22); among those reporting, 54.5% (12/22) were Stage 5–7, 13.6% (3/22) Stage 3–4 and 31.8% (7/22) Stage 1–2
- In our survey sample, organisations with ≥5 WTE clinical informaticians more often report stronger digital maturity indicators; however, EMRAM™ score reporting is limited to a subset of respondents and causality cannot be inferred.
- There is a relationship among workforce investment, EPR vendor choice, and EMRAM™ scores in our survey sample.
- Board-level visibility remains variable; 61.1% (55/90) reported the clinical informatics team reports directly to board-level leadership.
- Funding was the most commonly reported barrier (87.2%, 68/78), alongside staffing shortages (56.4%) and legacy constraints.
The suggested action items from the findings include national minimum workforce standards, equitable leadership banding, strengthened multidisciplinary inclusion, improved governance visibility and delegated authority, sustained investment, and repeatable benchmarking.
Overall, the survey findings suggest digital maturity is driven not only by technology adoption but by sustained investment in people, governance, and inclusive leadership (Faculty of Clinical Informatics, 2022; NHS England, 2021; Topol, 2019; Wachter, 2016).
