Improving Heart Failure Care through Digital Patient Engagement

Chang Gung Memorial Hospital (CGMH) in Linkou implemented a digital heart failure care model to reduce readmissions and improve patient outcomes. The program uses the Chiline Health Management Platform to remotely monitor patients’ blood pressure, heart rate, and weight after discharge, enabling case managers and clinicians to intervene early and adjust treatment in real time.

Since implementation, CGMH has seen near-zero 30-day readmission rates, improved medication adherence, and higher patient satisfaction, which all contribute to better clinical and financial outcomes. This successful initiative earned CGMH recognition as a 2025 HIMSS Davies Award winner.
